Server Query Updates

I've finished the updates to the server query, including performance improvements, template changes including HTML fixes and field checking, and increased information in some cases.

The demo site now shows examples of UT3, UT2004, and UT '99 servers:
http://www.utstatsdb.com/demo/

The updated versions will be included in the next UTStatsDB release.

The changes will be included in the upcoming 3.06 release. The work-in-progress is available via SVN, but I recommend this only for testing.
